It was handcrafted by Winfield Ware of Santa Monica California.  I inherited this piece from my mother and know that it has to be at least pre-1970 if not before.  It is in excellent condition with no chips, cracks or crazing.  The dish measures approximately 5" wide, 2" tall and 4-3/4" tall including the handle.
